Air Filtration and Ventilation Systems


  • Communication and Signage

  • Is there signage about (1) maximum occupancy, and (2) where to file a complaint, posted both indoors and outside?

  • Is the required Safety Plan on-site available for review?

  • Is there signage at the site to remind personnel to adhere to proper hygiene, social distancing rules, appropriate use of PPE, and cleaning and disinfecting protocols?

Physical Distancing


  • Is the current number of patrons at or below the maximum occupancy posted?

  • Are tables limited to 10 patrons?

  • Are tables spaced 6 feet apart, or separated by barriers at least 5 feet tall?

  • Is there a designated area for pickups and deliveries?

  • Are there markings 6 feet apart where people would be forming lines or gathering? 

Personal Protective Equipment (PPE) 


  • Are all employees wearing face coverings?

  • Is there a supply of replacement face coverings for employees?

Screening and Data Collection for Employees, Vendors, and Patrons


  • Are thermometers being used to screen indoor dining patrons? (Or, if no patrons currently onsite, are thermometers available for screening?)

  • Is the business collecting contact information (name, address, and phone) for one person from each party?

  • Is the business storing the collected contact information for 28 days?
